Client Intake Form
Tired of having your client notes scattered on different notepads, post-its and scrap paper lying around? (If you're like me, sometimes those notes don't even make it from your brain onto paper...) I designed this Client Intake Form to help gather all of the info I needed for my wedding stationery clients in one place. From the wedding date, to the planner and photographer info, important deadlines and all items that would be included in the stationery package as well as the add-ons, even timeline checklists... It's all in one handy dandy booklet you can print at home! (With plenty of designated spaces for notes).
- The 1st page is the front/back of the booklet (shown). The front is the landing page with all necessary info about your client and their wedding day. The backside is for any Day-Of items.
- The 2nd page is the inside of the booklet (not shown, because I can't give you the milk for free). The inside pages include a side for Save the Dates and a side for Formal Invitations.
How do I print?
This form is designed to be printed double-sided on Tabloid size paper (11x17) and folded in half. Folds perfectly to 8.5x11. If you do not have double-sided print capability on your printer, simply print the 1st page and then flip the paper around, put it back in your printer tray and print on the other side!
Bam- you just got more organized.
